No,
it is not possible to have a checkbox form field inside a document whose state
bound to a database mergefield. However during runtime we can change the state
of a checkbox based on a database value. We have prepared a simple sample to
illustrate the same. Kindly download the sample from the attachment and let us

Regarding
the Checkbox form field to survive the merge process : Could you please provide
us the input template document and your expected output document, so that we
can better understand your requirement and update you with appropriate solution.
Note:
Mailmerge process does not affect document formatting’s, it only changes the
values of the mergeFields. We have illustrated the same in the given sample.
