Catastrophic failure

This happens when I try to use the Code Generator.  I am running version 19.4.0.50.  The other extensions are also at this level.  The error occurs when clicking the Insert Button ( after filling out the other fields).  The error occurs whether or not I have selected Read-Only Columns
